Clinical Nurse Specialist/Therapist - Acute Frailty

We are seeking a highly skilled Clinical Specialist Nurse/Therapist to join our team in Acute Frailty. The successful candidate will have NMC/HCPC registration and a passion for delivering high-quality care to patients.

Main Duties of the Job

The successful candidate will work autonomously to provide advanced assessment, clinical diagnosis, and treatment of patients within the acute frailty pathway. This will involve working primarily within the emergency care floor and in-reach into acute wards, liaising with acute therapy and nursing teams to assess needs and provide same-day services.

The successful candidate will also support the delivery of frailty awareness activities across acute services and deputise for the Acute Frailty Consultant Practitioner in operational leadership and clinical governance.

Person Specification
  • Physiotherapy/Occupational Therapy Degree/Diploma.
  • HCPC Registration.
  • Evidence of CPD at a postgraduate level, reflective practice, IST.
  • Experience of working as part of a therapy team/multidisciplinary team.
  • Extensive general and acute experience.

  • Demonstrated ability to complete complex physical skills involving the moving and handling of patients and equipment.
  • Demonstrated ability to exert moderate physical effort for several short, and occasionally long, periods during the working day.
  • Demonstrated effective performance and a clinical leader.
  • Demonstrated commitment to lifelong learning and application in practice.
  • Demonstrated ability to read, understand, and apply complex policy and protocols.

  • Demonstrated ability to effectively analyse professional and ethical issues.
  • Demonstrated ability to effectively manage own caseload and determine priorities.
  • Demonstrated ability to work autonomously, independently, and under pressure.
  • Demonstrated effective leadership skills including the ability to organise, prioritise, and delegate quickly and efficiently.
  • Demonstrated ability to problem solve and evaluate complex clinical situations.
Communication Skills
  • Demonstrated skills in communicating highly complex and sensitive information where there may be issues relating to how and what to convey, or barriers to understanding.
  • Demonstrated highly effective verbal and non-verbal communication skills.
  • Demonstrated ability to collate information and write complex clinical and non-clinical reports.

The successful candidate must have appropriate UK professional registration. This post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service.
