About the Role

We are seeking a highly motivated and experienced Mental Health Practitioner to join our team in the Mole Valley Community Mental Health Recovery Service (CMHRS). As a key member of our multidisciplinary team, you will play a vital role in providing high-quality care and support to individuals with severe and enduring mental health needs.

The successful candidate will have a strong clinical background, excellent communication skills, and the ability to work effectively in a fast-paced environment. You will be responsible for assessing new referrals, developing and implementing individualized care plans, and providing psychological interventions and support to service users.

Key Responsibilities
  • Develop and implement individualized care plans to support recovery and independence
  • Provide psychological interventions and support to service users
  • Assess new referrals and develop effective treatment plans
  • Work collaboratively with other professionals to coordinate care and support
  • Contribute to the development and implementation of service-wide initiatives and policies
Requirements
  • NMC Registration as RN3 or RNMH
  • Clinical nurse qualification at Diploma or degree level and membership of a professional body
  • Minimum 5 years' experience working with people with mental health needs
  • Experience of leading and adapting to change within area of practice
  • Proven track record of delivering on performance and promoting excellence in practice
