About the Role

We are seeking a highly motivated and experienced Community Mental Health Practitioner to join our team in the Mole Valley Community Mental Health Recovery Service (CMHRS). The successful candidate will work with individuals who experience severe and enduring mental illness, providing evidence-based treatment and interventions to promote recovery and independence.

The ideal candidate will have a strong understanding of community mental health care and be able to work effectively with individuals, families, and carers. They will also have excellent communication and interpersonal skills, with the ability to work collaboratively with other professionals to coordinate care under the Care Programme Approach.

This is an exciting opportunity to work in a dynamic and innovative service, with a focus on recovery and promoting independence. The successful candidate will be supported in their role through formal supervision and training, as well as a strong multidisciplinary and collaborative approach within the team.

Main Responsibilities
  • Assess new individuals referred to the service and work with an allocated caseload of existing service users
  • Provide low-level psychological interventions and liaise with other professionals to coordinate care
  • Safely administer medication in a community setting and engage in medicines management and physical health monitoring
  • Provide educational input and supervision to nursing students and junior colleagues

Person Specification
  • Experience working in primary or secondary community mental health care
  • Able to carry out supervision
  • NMC Registered Nurse, OT or Social worker Registration
  • Successful completion of Mentorship course
