Job Title: Children's Community Nurse

We are seeking a highly skilled and experienced Children's Community Nurse to join our team at Guy's and St Thomas' NHS Foundation Trust. As a key member of our community nursing service, you will be responsible for providing high-quality care to children and young people with ongoing conditions such as asthma, eczema, and constipation.

About the Role

As a Children's Community Nurse, you will be responsible for assessing, planning, and delivering specialist nursing care to children and young people in various settings, including homes, schools, and community clinics.

Key Responsibilities
  • Provide high-quality specialist nursing care to children and young people with ongoing conditions
  • Assess and plan care for children and young people in various settings
  • Deliver education and training to children and young people, families, and healthcare professionals
  • Collaborate with other healthcare professionals to ensure seamless care
  • Contribute to service development and quality improvement initiatives
Requirements

To be successful in this role, you will need to have:

  • Experience of working as a nurse in a community or primary care setting
  • Excellent communication and interpersonal skills
  • Ability to work autonomously and as part of a team
  • Knowledge of clinical governance and risk management principles
  • Ability to use IT systems to manage patient care
