Job Title: Patch Children's Community Nurse

We are seeking a highly skilled and experienced Children's Community Nurse to join our team at Guy's and St Thomas' NHS Foundation Trust. As a key member of our Patch Children's Community Nursing (CCN) team, you will play a vital role in providing high-quality care to children and young people with ongoing conditions such as asthma, eczema, and constipation.

About the Role

This is an exciting opportunity to work in a dynamic and innovative team that is making a real difference in the lives of children and young people. As a Children's Community Nurse, you will be responsible for assessing, planning, and delivering specialist nursing care to children and young people in various settings, including homes, schools, and community clinics.

Key Responsibilities
  • Provide high-quality specialist nursing care to children and young people with ongoing conditions.
  • Work collaboratively with other healthcare professionals to ensure seamless care delivery.
  • Develop and implement individualized care plans to meet the unique needs of each child or young person.
  • Conduct clinical assessments and provide expert nursing advice to parents, carers, and other healthcare professionals.
  • Engage in research and quality improvement initiatives to enhance the quality of care and services.
Requirements

To be successful in this role, you will need to have:

  • Registered Nurse (Child) qualification and current registration with the NMC.
  • Experience of working with children and young people with ongoing conditions.
  • Excellent communication and interpersonal skills.
  • Ability to work autonomously and as part of a team.
  • Strong organizational and time management skills.
