This is an exciting opportunity for a seasoned Rehabilitation Case Manager to join Everpool as a Team Lead. Reporting directly to the Operations Director, you will be responsible for managing a team of Case Managers, ensuring seamless service delivery and exceptional client outcomes.

About the Role:

  • The ideal candidate will possess significant case management experience, with a proven track record of delivering high-quality services.
  • You will be based in Southeast, Central, and Greater London, with travel requirements across the South region.
  • A hybrid work schedule will be adopted, comprising both remote and field-based responsibilities.
  • This is a full-time position, offering a competitive salary of up to £60,000, inclusive of a car allowance.
  • Working hours will be Monday – Friday, between 8am-6pm, with flexible arrangements available for individual preferences.

Key Responsibilities:

  1. As a Team Lead, you will utilize your expertise in case management to provide effective support to individuals, developing and implementing tailored rehabilitation plans that address client needs and goals.
  2. Establishing and maintaining strong relationships with clients, their families, and relevant stakeholders is crucial to this role.
  3. You will conduct regular reviews and assessments to ensure optimal progress and outcomes, collaborating with multidisciplinary teams to facilitate seamless service delivery.
  4. Staying up-to-date with industry developments, best practices, and regulatory requirements is essential for success in this position.

Requirements:

  • A minimum of 2 years of experience in case management is required for this role.
  • Strong understanding of relevant laws, policies, and regulations is necessary for this position.
  • Excellent communication, interpersonal, and problem-solving skills are essential for success in this role.
  • Ability to work autonomously and as part of a team is vital for this position.
  • Proficiency in using relevant software applications and technology is also required.
