Job Description
We are seeking a highly motivated and empathetic individual to join our team as a Support Worker. In this rewarding role, you will work closely with our service users to provide personalized care and support, promoting their independence, dignity, and well-being. Your key responsibilities will include:
- Delivering person-centered care and support to service users in their homes and communities
- Building strong relationships with service users, their families, and other stakeholders
- Collaborating with our multidisciplinary team to develop and implement individualized care plans
