Mental Health Support Specialist

The Mental Health Support Coordinator (MHSC) Service is a collaborative effort between Primary and Secondary Clinical Services, as well as other key providers in the area, aimed at ensuring seamless pathways for clients, particularly those facing complex situations. This service brings together the PCN, Southdown, and Sussex Partnership NHS Foundation Trust, working in partnership through a multidisciplinary team approach.

  1. Provide short-term support to a caseload of clients with complex needs, utilizing a strength-based and person-centered approach to help them engage and achieve their desired outcomes.
  2. Enhance patient wellbeing by increasing access to non-clinical services using motivational and coaching techniques.
  3. Facilitate smooth pathways into services through advocacy, effective communication, and collaborative working with services.
  4. Promote and support the implementation and development of the service by building and maintaining professional relationships in GP practices and attending relevant meetings.

To succeed in this role, you will require an exceptional understanding of complex mental health needs and wellbeing, along with experience supporting this client group or equivalent transferable skills, and a genuine passion for helping people explore their needs. You will receive a comprehensive induction followed by regular training – we will invest in you to provide all the tools you need to succeed.
