Children's Complex Care and Continuing Care Clinical Manager

We are seeking an experienced and innovative nurse to lead and manage our children's continuing care and commissioning team. As a key member of our team, you will be responsible for developing and implementing strategies to improve the health and wellbeing of children and young people with complex needs.
Main Responsibilities- Lead and manage the children's continuing care and commissioning team, ensuring that all aspects of care are delivered to the highest standard.
- Develop and implement policies and procedures for children's continuing care, ensuring that they are aligned with national guidelines and best practice.
- Work closely with healthcare professionals, social workers, and other stakeholders to ensure that children and young people receive the best possible care and support.
- Manage and coordinate the commissioning of services for children and young people with complex needs, ensuring that they receive the right care and support at the right time.
- Develop and maintain relationships with healthcare providers, social care services, and other stakeholders to ensure that children and young people receive seamless care and support.
- Monitor and evaluate the effectiveness of children's continuing care services, identifying areas for improvement and implementing changes as needed.
- Ensure that all aspects of children's continuing care are delivered in accordance with national guidelines and best practice.
