About the Role

We are seeking a highly skilled and experienced Senior Case Worker to join our Mental Health team in Hampshire. As a Senior Case Worker, you will play a vital role in assessing the needs of adults in the community and providing them with support and guidance to maximise their wellbeing.

Key Responsibilities
  • Assess the needs of adults in the community and develop person-centred plans to support their wellbeing.
  • Provide coaching and mentoring support to Case Workers and contribute to the development of the team.
  • Collaborate with individuals, their families, providers, and other healthcare professionals to ensure effective support and care.
  • Use your strong relationship management and negotiation skills to build and maintain effective relationships with stakeholders.
Requirements

To be successful in this role, you will need:

  • A strong understanding of strengths-based practice and the ability to apply this in a community setting.
  • Excellent communication and interpersonal skills, with the ability to build and maintain effective relationships with individuals and stakeholders.
  • Experience of working in a community setting, with a focus on supporting adults with complex needs.
  • A current driver's licence and access to a reliable vehicle.
