Job Summary

We are seeking a highly skilled and experienced Senior Case Worker to join our Mental Health team in Hampshire. As a Senior Case Worker, you will play a key role in assessing the needs of adults in the community and providing them with support and guidance to maximise their wellbeing.

Key Responsibilities
  • Assess the needs of adults in the community and develop person-centred plans to support their wellbeing.
  • Provide coaching and mentoring support to Case Workers and contribute to the development of the team.
  • Collaborate with individuals, their families, providers, and other healthcare professionals to ensure effective support and care.
  • Use strengths-based practice to focus on people's resourcefulness and abilities, enabling them to develop strategies to face life's challenges and live independently and safely in the community.
Requirements

To be successful in this role, you will need:

  • Strong relationship management and negotiation skills.
  • Experience of working with adults in the community, particularly those with complex needs.
  • Knowledge of strengths-based practice and person-centred approaches.
  • Ability to work flexibly and provide coaching and mentoring support to colleagues.
