About the Role:
Hampshire County Council is seeking a highly skilled and experienced Senior Case Worker to join our Mental Health team. As a Senior Case Worker, you will play a vital role in assessing the needs of adults in the community and providing them with support and guidance to maximize their wellbeing.
Key Responsibilities:
- Assess the needs of adults in the community and develop personalized support plans to promote their wellbeing.
- Work collaboratively with individuals, their families, providers, and other healthcare professionals to ensure comprehensive support.
- Provide coaching and mentoring support to Case Workers to enhance their skills and knowledge.
- Support complex cases and contribute to the development of effective solutions.
Requirements:
- Strong relationship management and negotiation skills to build trust and rapport with individuals and stakeholders.
- Excellent communication and interpersonal skills to work effectively with diverse groups.
- Ability to work flexibly and adapt to changing circumstances.
- Current drivers' license and access to a reliable vehicle.
