Responsibilities:
• Communicate with disabled children and their families to understand their needs and concerns
• Carry out assessments and safeguarding risk assessments to ensure the well-being of disabled children
• Develop and implement personalized care plans to support disabled children and their families
• Collaborate with other professionals to ensure seamless delivery of care and support

Requirements:
• Post-qualifying experience as a Social Worker
• SWE registration
• At least 1 year of UK experience
• Enhanced DBS check
• Eligibility to work in the UK
