About the Role

This is an exciting opportunity to join the Dorset Clinical Commissioning Group as a Community Children's Nurse – Child Health UHD@home Specialist. As a key member of our team, you will play a vital role in delivering high-quality care to children and their families in the comfort of their own homes.

Key Responsibilities

  • Deliver comprehensive nursing care to children with complex health needs, working closely with families and other healthcare professionals to develop and implement personalized care plans.
  • Conduct thorough assessments of children's health needs, identifying areas for improvement and developing strategies to address these.
  • Provide education and support to families on managing their child's health condition, promoting self-care and independence.
  • Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
  • Participate in quality improvement initiatives to enhance the quality of care and services provided.

Requirements

  • Good standard of general education, including literacy and numeracy.
  • Completion of a competency-based workbook or equivalent relevant experience to vocational level 3.
  • Ability to adapt and learn in a healthcare environment.
  • Previous experience of working within a care environment or NHS.
  • Experience of working as part of a multidisciplinary team.
  • Experience of working with the public.

Desirable Criteria

  • Short courses to undertake patient and clinical care duties.
