About the Role
This is an exciting opportunity to join the Dorset Clinical Commissioning Group as a Community Children's Nurse – Child Health UHD@home Specialist. As a key member of our team, you will play a vital role in delivering high-quality care to children and their families in the comfort of their own homes.
Key Responsibilities
- Deliver comprehensive nursing care to children with complex health needs, working closely with families and other healthcare professionals to develop and implement personalized care plans.
- Conduct thorough assessments of children's health needs, identifying areas for improvement and developing strategies to address these.
- Provide education and support to families on managing their child's health condition, promoting self-care and independence.
- Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
- Participate in quality improvement initiatives to enhance the quality of care and services provided.
Requirements
- Good standard of general education, including literacy and numeracy.
- Completion of a competency-based workbook or equivalent relevant experience to vocational level 3.
- Ability to adapt and learn in a healthcare environment.
- Previous experience of working within a care environment or NHS.
- Experience of working as part of a multidisciplinary team.
- Experience of working with the public.
Desirable Criteria
- Short courses to undertake patient and clinical care duties.
