Job Description:

Type: Full-time, hybrid (around 50% from home, 50% at GP surgeries)
We are looking for pharmacists to join our team to support patients with chronic conditions in primary care settings. This role will allow you to utilise your clinical expertise and commercial acumen to enhance the awareness of Interface Clinical Services within primary care and assist in developing long-lasting relationships with practices and local NHS organisations.

Key Responsibilities:

  • Support Practices to provide clinical tools and resource to help manage patients with long-term conditions.
  • Enhance the awareness of Interface Clinical Services within primary care and assist in developing long-lasting relationships with practices and local NHS organisations.
  • Community or primary care experience desirable.
  • Willing to travel as required, along with remote (home) working.
  • A keen passion to make a positive difference to patients' long-term health.

Estimated Salary: £60,000 - £80,000 per annum depending on location and experience.
