Key Responsibilities:

  3. Coordinate services with multiple healthcare providers to ensure a cohesive approach to care.
Care Coordination:
  1. Establish effective relationships with healthcare professionals to ensure comprehensive care delivery.
  2. Communicate regularly with patients and families regarding care updates, changes, and instructions.
Monitoring and Evaluation:
  1. Continuously monitor patient progress and adjust care plans as necessary.
  2. Evaluate patient outcomes to ensure care meets clinical standards and best practices.
Support and Advocacy:
  1. Serve as the primary point of contact for patients and families, providing support and education on the care process.
  2. Help patients understand treatment options, insurance coverage, and rights.
Documentation and Reporting:
  1. Accurately record patient interactions, care plans, and medical history.
  2. Ensure compliance with healthcare regulations, patient privacy laws, and organizational policies.
Collaboration:
  1. Work collaboratively with healthcare team members to deliver high-quality patient care.
  2. Participate in team meetings to discuss patient care progress and share insights.

We are offering a salary of $62,000 per annum, plus benefits and opportunities for career advancement and personal growth.
