Job Title: Community GP

We are seeking a skilled and compassionate Community GP to join our team at North East Derbyshire Primary Care Network Limited. As a Community GP, you will play a vital role in providing high-quality care to our housebound patients and those in care or residential facilities across our network.

About the Role

This is an exciting opportunity for a motivated and experienced GP to lead our multi-disciplinary team in delivering proactive, personalized, and integrated health and social care to our local population. You will be responsible for developing continuity relationships with our housebound patients, taking a holistic approach to their management, and liaising with other health and social care organizations to improve the care of these vulnerable individuals.

Key Responsibilities
  • Provide day-to-day clinical leadership for the multi-disciplinary team, including debriefing after visits as needed.
  • Home visiting for clinically complex cases, end-of-life care, diagnostic uncertainty, and complex prescribing decisions.
  • Senior clinical triage for the Acute Home Visiting service.
  • Build relationships across the extended team, including social care, ambulance, social prescribers, community, and mental health clinicians.
  • Opportunity for involvement in service development as the team expands.
  • Involvement in MDT meetings.
  • Provide holistic comprehensive assessment and enhanced advance planning.
  • Maximize best care in the patient's own homes to reduce the need for hospital or care home admissions.
  • Provide end-of-life care in line with the patient's wishes.
  • Quality improvement activity.
  • Opportunities for teaching and mentorship.
