We are seeking a highly skilled and dedicated Mental Health Community Practitioner to join our team at Herefordshire and Worcestershire Health and Care NHS Trust. As a key member of our Community Mental Health Service, you will play a vital role in delivering evidence-based interventions and providing high-quality care to patients with mental health problems.
Main Responsibilities- Work as a key member and resource of the MDT core team, providing prompt local assessment, triage, risk assessment, risk management, signposting, onward referral, and evidence-based interventions for people presenting with mental health problems.
- Represent the team on specific projects and working groups as required, utilizing your expertise and knowledge to drive positive change and improvement within the service.
- Design and deliver evidence-based recovery-focused mental health interventions and groups, utilizing your skills and experience to promote positive outcomes and well-being for patients.
- Work collaboratively with colleagues, partners, and stakeholders to develop and maintain effective relationships, ensuring seamless communication and coordination of care.
- Attend weekly MDT meetings, participate in professional meetings, and signpost to adjunct services depending on patient need, ensuring that patients receive comprehensive and coordinated care.
