Job Summary

We are seeking a highly skilled and dedicated Mental Health Community Practitioner to join our team at Herefordshire and Worcestershire Health and Care NHS Trust. As a key member of our Community Mental Health Service, you will work closely with patients, families, and other healthcare professionals to provide high-quality care and support.

Main Responsibilities
  • Assess and manage patients with mental health problems, providing evidence-based interventions and support.
  • Work collaboratively with the multidisciplinary team to develop and implement care plans.
  • Provide education and training to patients, families, and carers on mental health issues and recovery-focused interventions.
  • Participate in regular team meetings and contribute to the development of service policies and procedures.
  • Represent the team on specific projects and working groups as required.
