Mental Health Practitioner

As a Mental Health Community Practitioner at Herefordshire and Worcestershire Health and Care NHS Trust, you will be working as a key member of the multi-disciplinary team to deliver evidence-based interventions for people presenting with mental health problems. Your role will involve working with patients, carers, and other healthcare professionals to provide assessment, triage, risk assessment, risk management, signposting, onward referral, and evidence-based interventions.
Main Responsibilities
- Work across all clinically appropriate patients, 17-years-old and over, to deliver evidence-based interventions.
- Provide prompt local assessment, triage, risk assessment, risk management, signposting, onward referral, and evidence-based interventions for people presenting with mental health problems.
- Represent the team on specific projects and working groups as required.
Requirements
- Relevant professional qualification relating to Health or Social Care in social work, nursing, OT with current professional registration.
- Recent evidence of continuing professional development.
- Ability to meet the travel requirements of the post.
