About the Role
We are seeking a skilled Mental Health Community Practitioner to join our team at Herefordshire and Worcestershire Health and Care NHS Trust. As a key member of our Community Mental Health Service, you will work closely with patients, colleagues, and partner agencies to deliver high-quality care and support.
Key Responsibilities
- Assess and manage clinical risk for patients presenting with mental health problems.
- Work collaboratively with the multi-disciplinary team to provide prompt local assessment, triage, and onward referral.
- Deliver evidence-based interventions and groups to support patient recovery and well-being.
- Represent the team on specific projects and working groups as required.
Requirements
- Relevant professional qualification in mental health nursing, occupational therapy, or social work.
- Experience of working in a mental health setting, including assessment and management of clinical risk.
- Ability to work flexibly and manage a blended caseload of common mental health disorders.
