Therapy Support Worker

We are seeking a highly motivated and dedicated Therapy Assistant to join our integrated therapy team on Zetland Ward in Redcar Primary Care Hospital. As a key member of our team, you will work closely with Physiotherapists, Occupational Therapists, and nurses to provide rehabilitation to patients on the ward, promoting independence and supporting a safe discharge.
Main Responsibilities- Provide high-quality patient care and support to patients with a range of medical, social, and functional problems.
- Work as part of a team to deliver rehabilitation programs, including physiotherapy, occupational therapy, and nursing care.
- Develop and maintain strong relationships with patients, families, and healthcare professionals to ensure seamless care coordination.
- Participate in service developments and clinical governance initiatives to improve patient outcomes and service delivery.
- Contribute to the development and implementation of individualized care plans, ensuring patient-centered care and optimal outcomes.
- 3 A-levels or equivalent level 3 qualification(s) and Functional Skills (level 2) or a level 3 apprenticeship, such as the Senior Healthcare Support Worker apprenticeship.
- Experience of working in health or social care, with a strong understanding of patient-centered care and rehabilitation principles.
- Basic IT skills to use electronic patient records and other Trust systems.
- Ability to work independently and as part of a team, with excellent communication and interpersonal skills.
