Support Worker, Reablement Team Facilitator

The Reablement Team offers a unique opportunity to support individuals in living independently at home. Our short-term service helps people develop confidence and skills necessary for self-sufficiency. With an impressive 80% success rate, we've enabled most service users to remain independent without formal support. We're proud to be part of a highly commended team for improving value in care delivery for frail older patients and integrated discharge planning and reablement.

The Reablement Support Worker position involves working within an integrated Reablement Team and multidisciplinary teams to provide outcome-focused support to service users in their homes and communities. Your primary goal will be to help individuals regain, improve, and maintain skills for daily living, promoting maximum independence.
Your key responsibilities will include assessing service users' needs, monitoring progress against care plans, liaising with other professionals, and participating in mental and physical health monitoring. You'll also assist with tasks such as bill management, shopping, and prescription collection, as well as contributing to the promotion of positive links with local community agencies and groups.
In addition to these duties, you'll work with vulnerable service users to develop, regain, and maintain skills that maximize their independence. This may involve personal care tasks, always treating them with dignity and respect. You'll maintain accurate records and report any incidents or concerns, ensuring compliance with relevant legislation and local policies.
We're seeking a highly motivated and enthusiastic individual who is dedicated to providing exceptional service to the residents of Swindon. You'll need to be able to work independently, using initiative, and as part of the wider Reablement Team.
Essential qualifications and experience include caring for vulnerable adults with complex needs, including personal care, and working in community or care settings. Travel throughout the borough is essential, and you'll require a full driving license and access to a vehicle during working hours. Fluency in English is mandatory due to the requirements of Part 7 of the Immigration Act.
