About the Role:

We are seeking an experienced Specialist Learning Disabilities Practitioner to join our well-established team in London. As a dedicated professional, you will play a vital role in providing high-quality care and support to individuals with learning disabilities.

Key Responsibilities:

  • Manage complex caseloads and provide person-centred support to service users
  • Conduct thorough assessments and reviews to ensure optimal outcomes
  • Collaborate with other agencies and professionals to deliver seamless services
  • Promote autonomy, choice, and control for service users
  • Support individuals to live safely and independently in their own homes

Requirements:

  • Post-qualifying experience as a Learning Disabilities Social Worker
  • HCPC Registration
  • 1-2 years' post-qualifying experience within the UK Social Care industry
  • Enhanced DBS/CRB clearance (or willingness to obtain)
  • Eligibility to work in the UK
