The North Staffordshire Combined Healthcare NHS Trust is a leading provider of mental health, social care, learning disability and substance misuse services in the West Midlands.

We are seeking a motivated and enthusiastic qualified nurse to join our team as a Complex Case Worker. This 12-month fixed term position will provide an exciting opportunity for you to work on the Transforming Care Partnership programme (TCP) alongside our multidisciplinary team.

The purpose of this role is to support the commissioning of safe and effective services for people with a learning disability, preventing hospital admission and ensuring individuals lead healthy, valued lives.

As a member of the Learning Disability Transforming Care Team, you will participate in the assessment, planning, and implementation of proactive care and case management for people with learning disabilities and/or autism in Staffordshire and Stoke-on-Trent.

You will be responsible for exercising independence, authority, and autonomy to inform practice, together with consultation and supervision, making properly informed independent decisions.

In addition to your clinical expertise, we value your ability to work collaboratively with multi-agencies and professionals, keeping abreast of changes and developments in national policy and legislation pertinent to the service area.
