Clinical Pharmacist Specialist

We are seeking a skilled Clinical Pharmacist Specialist to join our team at North Cotswold Primary Care Network. The successful candidate will play a key role in ensuring the highest quality, safety, and value of prescribing within our network.
The estimated annual salary for this position is £60,000 - £80,000 depending on experience. Our network is located in the beautiful rural setting of North Cotswolds, serving approximately 31,000 patients.
Key Responsibilities:- Undertake clinical medication reviews with patients to optimise their medication regimens.
- Work closely with care home staff to improve the safety of medicines ordering and administration.
- Proactively manage patients with complex polypharmacy, particularly those who are elderly, housebound, or have multiple long-term conditions.
- Provide specialist expertise in medicines use to address public health and social care needs.
Required Qualifications and Skills:
- Mandatory Professional registration with the General Pharmaceutical Council.
- Undergraduate degree in pharmacy and registration with the General Pharmaceutical Council.
- Membership of the Royal Pharmaceutical Society and/or Faculty membership.
- In-depth therapeutic and clinical knowledge, as well as evidence-based medicine principles.
