Job Summary

We are seeking a highly motivated and compassionate Cancer Care Coordinator to join our clinical team. As a key member of our team, you will play a vital role in supporting patients and their families throughout their cancer journey.

Main Responsibilities

  • Provide administrative support to our clinical teams, ensuring seamless communication and coordination of patient care.
  • Assist with the facilitation and implementation of holistic needs assessments, ensuring patients receive comprehensive and personalized care.
  • Triaging phone calls and emails, using a risk assessment framework to prioritize patient needs.
  • Provide general information and support about cancer services, using Trust-approved literature.
  • Signpost patients and relatives to relevant services, such as Macmillan and District Nurses.
  • Attend relevant clinics and interact with patients and relatives, providing emotional support and guidance.
  • Participate in audit and evaluation of service delivery, identifying areas for improvement and change.

Requirements

  • GCSE English Language and Mathematics or equivalent qualification.
  • NVQ Level 3 or equivalent knowledge and experience.
  • Experience working within the oncology setting, with a focus on patient care and support.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a team.
  • Ability to manage and prioritize personal workload, using own initiative and demonstrating effective organization and time management skills.
