Community Health Specialist

We are seeking an experienced Health Visitor to join our team in Northamptonshire. As a key member of our West Northants 0-19 Health Visiting Team, you will play a vital role in assessing and identifying health needs for individuals, families, and communities.
Your primary responsibility will be to work with vulnerable families, ensuring the voice of the child is heard and advocated for in multi-agency planning. You will also be responsible for delivering high-quality care, working closely with other healthcare professionals to achieve optimal outcomes.
The successful candidate will have a passion for providing increasingly complex support for families, ensuring they receive the care they need. With a strong focus on quality care, you will work collaboratively with our integrated skill mix team to deliver quality outcomes and improve the experience of families.
A relocation package of up to £8,000 is available, subject to terms and conditions. You will also have access to excellent learning and development opportunities, flexible working arrangements, and a generous pension scheme.
