About the Role

We are seeking an experienced Adult Services Professional to join our team as a Community Health Worker - Patient Support Specialist. This is a fantastic opportunity for someone who is passionate about delivering high-quality patient support services in a primary care setting.

As a Community Health Worker, you will work closely with our Primary Care Network (PCN) to provide person-centred support to patients, promoting their independence and well-being. You will be responsible for building trust with patients, providing non-judgmental support, and respecting diversity and lifestyle choices.

The ideal candidate will have experience of working directly in a community development context, adult health and social care, learning support or public health/health improvement. You will also have the ability to maintain effective working relationships and promote collaborative practice with all colleagues.

Key Responsibilities:

  • Providing person-centred support to patients, promoting their independence and well-being
  • Building trust with patients, providing non-judgmental support, and respecting diversity and lifestyle choices
  • Working collaboratively with the PCN to deliver high-quality patient support services
  • Maintaining accurate records and reporting to senior staff members as required

Requirements:

  • NVQ Level 3, Advanced level or equivalent qualifications or working
  • Experience of working directly in a community development context, adult health and social care, learning support or public health/health improvement
  • Ability to maintain effective working relationships and promote collaborative practice with all colleagues
