This is an exciting opportunity for a Healthcare Professional - Outreach to join the newly established Learning Disability and Autism (LDA) Outreach Team at Devon Partnership NHS Trust. As a key member of the team, you will work collaboratively with other LDA Outreach colleagues, community learning disability and autism colleagues, and colleagues from 'The Brook' our 10-bedded Learning Disability and Autism inpatient unit.

Key Responsibilities
  • Triage, assess, plan and deliver complex care packages to admission-vulnerable service users across Devon and for those discharged from The Brook.
  • Work proactively with other partner agencies, families and care providers at every stage of care.
Requirements

Post holders will require extensive clinical knowledge, advanced clinical leadership skills and excellent safeguarding, Mental Health Act and Mental Capacity Act knowledge. They should also have the ability to prioritize and manage workload, and excellent interpersonal, communication, formulation and risk assessment skills.
