Your focus will be on addressing the non-medical needs of adults who find themselves relying on Emergency Department visits.

Key Responsibilities:

Engage in direct interactions, either at home or within the community, to understand and address the unique needs of individuals. Initiate support through meaningful conversations centered around the individual's priorities, ensuring a personalized care approach. Collaborate with other services and resources to provide comprehensive support tailored to each person's situation.

Ideal Candidate Profile:

A solid understanding of health disparities and the broader factors influencing health outcomes. Proven experience as a case manager or practitioner, demonstrating a commitment to personalized care methodologies. Possession of a coaching certification or training in motivational interviewing, appreciative inquiry, or similar techniques. Experience in collaborative work and navigating various service systems effectively.
