About the Role

We are seeking a skilled Discharge Planning Nurse to join our team in the Neonatal Unit at University Hospital Southampton NHS Foundation Trust. This is an exciting opportunity to work in a dynamic environment where you will be responsible for ensuring that our patients receive seamless care throughout their journey.

The successful candidate will have a registered nurse or midwife qualification, evidence of ongoing personal development, and neonatal life support qualification. They should also possess excellent time management skills, ability to work effectively as a lone worker, and good communication skills.

As a key member of our team, you will work closely with colleagues to develop and implement individualised discharge plans that meet the unique needs of our patients and their families. You will also provide education and support to families, ensuring they feel confident and empowered to manage their care at home.

Estimated Salary: £35,000 per annum
