**About the Role**

We have an exciting opportunity for a skilled Children's Nurse to join our evolving Children's Hospital @ Home team. This role will involve working closely with University Hospital Southampton NHS trust (UHS), providing specialist nursing care, support, and co-ordination of care to children and young people with short-term acute healthcare needs requiring urgent care pathways.

The successful candidate will be responsible for facilitating early, safe planned discharge from hospital and preventing unnecessary admissions. This will involve working autonomously, making clinical decisions, and collaborating with multidisciplinary teams.

As a member of our team, you will have access to regular supervisions, 1:1s, and appraisals, as well as training and development opportunities to enhance your skills and career progression.

**Job Responsibilities**

  • Provide specialist nursing care, support, and co-ordination of care to children and young people with short-term acute healthcare needs.
  • Facilitate early, safe planned discharge from hospital and prevent unnecessary admissions.
  • Work autonomously, making clinical decisions, and collaborating with multidisciplinary teams.

**Requirements**

  • RN Child qualification.
  • Sound knowledge base in safeguarding and caseload management.
  • Ability to work independently and as part of a team.

**Salary Information**

The estimated salary for this role is £32,000 - £42,000 per annum, depending on experience.
