About the Role

We are seeking a highly skilled and motivated Adult Mental Health Practitioner to join our Older Peoples Mental Health Liaison Team in Stockport. As a key member of our team, you will work closely with our Urgent Community Response Team to provide specialist mental health nursing advice and joint assessments to individuals in crisis.

Main Responsibilities
  • Represent the Mental Health Liaison Service while working with the Urgent Community Response Team in Stockport.
  • Provide direct care to service users, supporting individuals in crisis due to physical health changes impacting their mental health.
  • Conduct thorough assessments of service users' mental health needs, including risk assessment, psychological needs, and cognitive functioning.
  • Make decisions regarding clinical management and critical clinical judgments in community settings.
  • Assess and manage clients with co-morbidities of physical and mental health needs, including medication and infection impact.

Requirements
  • Registered Mental Health Nurse (RMN) or Registered General Nurse (RGN) qualification.
  • Significant post-registration experience at Band 5 or above.
  • Experience working in older people's mental health inpatient, community, or liaison settings.
  • Knowledge of mental health assessment tools, current legislation, and safeguarding procedures.
