About the Role

We are seeking a highly skilled and compassionate Adult Mental Health Practitioner to join our Community Mental Health Team (CMHT) in Stockport. As a key member of our team, you will work closely with individuals to develop and implement personalized care plans, promoting recovery and well-being.

The CMHT is a dynamic and multidisciplinary team, comprising mental health nurses, occupational therapists, social workers, and support workers. We have access to psychology services and a commissioned Personality Disorder Pathway, enabling us to provide comprehensive care to our service users.

As a Mental Health Practitioner, you will be responsible for a defined caseload, working collaboratively with individuals to identify and address their needs. You will facilitate discharge from secondary mental health services, contribute to duty and clinic work, and participate in team development initiatives.
