About the Role

We are seeking a highly skilled and motivated Adult Mental Health Practitioner to join our Older Peoples Mental Health Liaison Team in Stockport. As a key member of our team, you will work closely with the Urgent Community Response Team to provide specialist mental health nursing advice and a joint assessment service.

Main Responsibilities
  • Represent the Mental Health Liaison Service while working with the Urgent Community Response Team in Stockport.
  • Provide direct care to service users in crisis, supporting individuals with physical health changes impacting their mental health.
  • Conduct thorough assessments of service users' mental health needs, including risk assessment, psychological needs, and cognitive functioning.
  • Make critical clinical judgments in relation to mental health and care provision within the community setting.
  • Assess and manage clients with co-morbidities of physical and mental health needs, including medication and infection management.

Requirements
  • Registered Mental Health Nurse (RMN) or Registered General Nurse (RGN) qualification.
  • Significant post-registration experience at Band 5 or above.
  • Experience of working in older people's mental health inpatient, community, or liaison settings.
  • Experience of implementing programs to manage Behavioral and Psychological Symptoms of Dementia (BPSD).
  • Experience of working with individuals in crisis and those with symptoms of mental health illness.
