We are seeking an experienced Lead Care Practitioner to join our team in [location]. Key Responsibilities

  • Provide leadership and guidance to care staff, ensuring they have the skills, knowledge, and confidence to deliver high-quality care.
  • Develop and implement care plans that meet the individual needs and preferences of our residents.
  • Work closely with the management team to monitor and improve the quality of care delivery, identifying areas for improvement and implementing changes as needed.
  • Collaborate with other healthcare professionals to ensure seamless care coordination and effective communication.
  • Mentor and supervise care staff, providing constructive feedback and coaching to support their professional development.

Requirements

To be successful in this role, you will need:

Qualified to NVQ Level 3 or Advanced Diploma in Health and Social Care.Previous experience in a residential care setting, with a proven track record of leadership and team management.Excellent communication and interpersonal skills, with the ability to work effectively with people at all levels.A commitment to delivering high-quality care and supporting the well-being of older people.
