Care and Support Specialist

Creative Support is a leading provider of person-centered support services for individuals with learning disabilities, autism, and mental health needs. We are seeking a highly motivated and caring individual to fill the role of Support Coordinator in our Oaklea House location.
Estimated Salary:£25,000 - £30,000 per annum, depending on experience.
Job Description:The Support Coordinator will provide outcome-focused support to enable clients to lead fulfilling and valued lives, maintain their tenancies, participate in their local community, and more. This will involve effectively meeting the needs of our clients, taking additional responsibilities such as rota management, induction of staff, and staff supervision, working in a flexible manner which may include working days, evenings, weekends, waking nights, and sleepovers.
Key Responsibilities:- Provide direct support to clients to help them achieve their goals and aspirations.
- Conduct assessments of client needs and develop proactive risk management strategies.
- Work collaboratively with colleagues to ensure effective service delivery.
