Children's Community Nurse

We are seeking a highly motivated and proactive Children's Community Nurse to join our friendly and progressive team. As a key member of our community children's nursing team, you will provide clinical nursing care, training, and support to children, young people, and their carers in the community.
The ideal candidate will have a strong background in community nursing, with experience in assessing and care planning. You will be confident, autonomous, and able to work independently, with a strong focus on patient safety and quality of care.
As a member of our team, you will contribute to the effectiveness of our services by providing skilled, evidence-based nursing care, working with patients and families in all stages of the care planning process, and acting as a patient advocate in the multi-disciplinary team.

Key Responsibilities:
- Providing clinical nursing care, training, and support to children, young people, and their carers in the community.
- Assessing and care planning for patients with complex needs.
- Working with patients and families in all stages of the care planning process.
- Acting as a patient advocate in the multi-disciplinary team.
Person Specification:
- Child branch/RSCN qualification.
- Evidence of continuous professional development.
- Previous experience as a band 5 or band 6 nurse.
- Assessing and care planning skills.
- Experience of safeguarding.
- Car driver and access to car.
