The Role
This is an exciting opportunity to build strong relationships with adults, professionals, and families. As a Care Coordinator, you will work collaboratively to plan and deliver personalized care plans that meet the unique needs of each individual. You will also participate in fun activities such as long walks, outdoor educational trips, and interactive hands-on activities.
You may be required to work independently at times, and as part of your role, you may assist with cooking and cleaning duties. A full UK driving license is preferred, but not essential.
Experience and Qualifications
We are looking for someone with previous experience supporting individuals with mental health issues. You should possess excellent communication and interpersonal skills, with the ability to build trust and rapport with our service users and their families.

Salary
The estimated salary for this role is approximately £25,000-£30,000 per annum, depending on experience and qualifications.
