About the Role

We are seeking a highly skilled and motivated Work and Health Coach to join our team at Viaduct Care CIC. As a Work and Health Coach, you will play a vital role in supporting individuals with health conditions to access employment opportunities and achieve their goals.

Key Responsibilities

  • Conduct initial assessments to identify barriers to employment and develop personalized action plans to support individuals in achieving their goals.
  • Provide one-to-one coaching and support to individuals to help them manage their health conditions and improve their employment prospects.
  • Build strong relationships with local employers and community organizations to promote employment opportunities and provide support to individuals.
  • Work collaboratively with a multidisciplinary team to deliver the WorkWell Pilot and achieve project targets.
  • Contribute to the development of education materials and participate in audit activities to improve service delivery.
  • Communicate effectively with individuals, employers, and other stakeholders to promote the service and provide support.

Requirements

  • Experience of using coaching approaches and frameworks to support individuals with health conditions.
  • Ability to work independently and as part of a team to achieve project targets.
  • Excellent communication and interpersonal skills to build strong relationships with individuals, employers, and other stakeholders.
  • Ability to work flexibly and adapt to changing circumstances.
  • Knowledge of the voluntary and social care sector and employment support services in Stockport.
