About the Role

We are seeking a highly skilled and motivated individual to join our team as a Work and Health Coach. As a key member of our team, you will play a vital role in supporting individuals with health-related barriers to employment.

Key Responsibilities
  • Conduct initial assessments to identify barriers to employment and develop personalized action plans.
  • Provide one-to-one support to individuals, empowering them to set goals and achieve employment and health outcomes.
  • Use a structured framework to coach individuals across a series of sessions, building skills and confidence to achieve goals.
  • Manage and prioritize a caseload, referring individuals to other health professionals or agencies as needed.
  • Develop strong links with local organizations to encourage referrals and promote the service.
  • Engage with employers to support employees with reasonable adjustments and promote support available to them.
  • Work as part of a multidisciplinary team to support the delivery of our services.
Requirements
  • Experience of using coaching approaches and frameworks, such as Motivational Interviewing.
  • Experience of working with diverse individuals with different backgrounds and needs.
  • Ability to work independently and as part of a team.
  • Proficient in Microsoft Office and web-based services.
  • Good communication and interpersonal skills.
  • Ability to deal with challenging behavior and difficult conversations.
