About the Role

We are seeking a skilled and compassionate Work and Health Coach to join our team at Viaduct Care CIC. As a Work and Health Coach, you will play a vital role in supporting individuals with health conditions to access employment opportunities and achieve their goals.

Key Responsibilities

  • Conduct initial assessments to identify barriers to employment and develop personalized action plans.
  • Provide one-to-one support to individuals, empowering them to manage their health and employment needs.
  • Build strong links with local employment and skills support services, community-based health provisions, and social prescribing services.
  • Manage a caseload of participants, prioritizing their needs and providing regular updates.
  • Collaborate with employers to provide reasonable adjustments and promote support services.

Requirements

  • Experience of using coaching approaches and frameworks, such as Motivational Interviewing.
  • Ability to work with diverse individuals, including those with long-term conditions.
  • Excellent communication and interpersonal skills, with the ability to build rapport and establish effective relationships.
  • Proficient in Microsoft Office and web-based services.
