**Job Overview**
We are seeking an experienced and motivated Occupational Therapist to join our team at NELFT NHS Foundation Trust. As a key member of our Integrated Community Team, you will play a vital role in providing high-quality patient-centered care to individuals with physical conditions, including neurological, orthopaedic, and musculoskeletal disorders.
**Key Responsibilities**
- Work closely with community nurses, community matrons, physiotherapists, and rehabilitation assistants to maintain independence, promote self-care, and assist patients in achieving their goals.
- Conduct thorough assessments and develop personalized rehabilitation plans to address the unique needs of each patient.
- Collaborate with multidisciplinary teams to ensure seamless care coordination and effective communication.
- Provide education and support to patients and their families to promote healthy lifestyles and independence.
**Requirements**
- Experience working at B5/B6 level in a healthcare setting.
- Previous NHS experience, preferably in community rehabilitation.
- Degree in Occupational Therapy or equivalent.
- HCPC registration.
