We are seeking a skilled Clinical Care Coordinator to join our team at Active Care Group Recruitment. This role offers an exciting opportunity to make a real difference in the lives of patients with neuro and mental healthcare needs.

The successful candidate will work in conjunction with a multidisciplinary team to formulate care plans, assess and manage risks, and regularly review risk factors. A current NMC registration as a Registered Nurse (RN), Mental Health Nurse (RMN), or Learning Disability Nurse (RNLD) is essential for this role.

Key responsibilities include being an active participant in the multidisciplinary team, undertaking clinical supervision, acting as a mentor/preceptor, and supervising junior staff in carrying out their care plans. The ideal candidate will have excellent communication skills, be able to work effectively with carers and relatives, and aspire to provide an effective, seamless, and integrated service to patients/residents.

A competitive salary of approximately £34,000 - £40,000 per annum, depending on experience, is offered, along with 25 days plus bank holidays, birthday off, and access to a wide range of e-learning and face-to-face training opportunities through the Active Learning Hub.
