We are currently seeking a highly skilled Complex Care Coordinator to join our team. As a Community Nurse Case Manager, you will play a vital role in delivering high-quality care to clients in the Birmingham region.

Role Description
  • Conducting initial assessments in the community
  • Training and assessing workers' competencies and allocating support workers to patients
  • Collaborating with the wider multidisciplinary team (MDT)
  • Attending meetings and making referrals
  • Managing a team of non-clinical case managers
Package and Benefits
  • An estimated annual salary of £46,000 per annum (£1,167 per week)
  • £3,000 car allowance
  • Mileage expenses
  • Full-time hours: 37.5 hours per week, Monday to Friday

To be considered for this role, please ensure you meet the following criteria:

  • You are an RGN or RSCN with advanced clinical skills in complex care
  • You have experience delivering care to both adult and paediatric service users
  • You are a car driver, flexible to travel around the Birmingham/West Midlands area
