**Job Summary:** Community Care Practitioner
**Estimated Salary:** £35,000 - £50,000 per annum
We are seeking a skilled Community Care Practitioner to join our team at North London NHS Foundation Trust. As a key member of our Core Community Mental Health Team, you will be responsible for delivering high-quality care services to patients, service users, and carers.
The successful candidate will participate in daily Core Mental Health MDT referral meetings, where new referrals will be screened and allocated to relevant Mental Health practitioners. You will also be responsible for carrying out practical tasks as part of service users' care plans and delivering interventions as part of an initial offer.
Key Responsibilities:
- Deliver care services to patients, service users, and carers
- Participate in daily Core Mental Health MDT referral meetings
