The role of a Community Support Worker with Derbyshire Community Health Services NHSFT involves providing support to patients in their own homes, focusing on optimising health and independence. The shift pattern is divided into Early (08:00-14:00) and Late (16:00-22:00) periods over a seven-day rota. A full-time salary of £28,000 per annum is offered, along with part-time options available.

Key Responsibilities:
  • Providing individual support without direct supervision in the community
  • Assisting clinicians in delivering agreed treatment and care programs where necessary
  • Maintaining high standards of patient care through knowledge and competency development

Our team provides community health services to over one million people in Derbyshire and Derby, working collaboratively as part of Joined Up Care Derbyshire. We employ approximately 4,200 staff members, caring for patients in various settings, including hospitals, health centres, clinics, and homes.

Person Specification:
  • NVQ Level III in care or equivalent qualification
