Role Overview
The position of Care Coordinator is a vital component of our team, providing a crucial service to our clients. As a Care Coordinator, you will be responsible for ensuring the provision of high-quality care services to vulnerable individuals living in their own homes.
Key Responsibilities
- Adhere to core values, policies, and procedures
- Apply diversity and equal opportunities policies in the workplace
- Interview prospective candidates for the role of carer
- Ensure all client details, care plans, and risk assessments are in place
- Plan to cover holidays, training, and planned absence
- Organize and plan care worker rotas, considering travel time and staff skill mix
- Answer the telephone promptly and professionally, dealing with enquiries or queries
- Ensure the provision of high-quality care services to vulnerable individuals living in their own homes
- Undertake staff supervisions and appraisals
- Liaise with other professionals, such as GPs, commissioners, and district nurses
- Work alongside the manager to ensure regular reviews of service users are completed in a timely manner
- Ensure all electronic rostering systems are up-to-date
- Submit travel expense forms within the set time frame
Essential Requirements
- Strong skills in planning, scheduling, prioritizing, and time management
- Minimum of 1 year's experience in a similar role
- Relevant qualification, such as Business Studies or Social Care (desirable)
- Driving license and access to own vehicle
- Extremely well-organized, excellent planning, and prioritizing ability with high attention to detail
- Self-motivated and flexible, with a willingness to participate in an on-call system for out-of-office hours
- Previous Domiciliary Care experience (desirable)
- Knowledge of office software, including Word and Excel
