Senior Social Care Specialist

We are seeking a Senior Social Care Specialist to join our team at Ackerman Pierce Ltd. The ideal candidate will have experience in providing care coordination approaches to manage and support individuals who meet the eligibility for Continuing Health Care (CHC).
Main duties include:
- Creating and carrying out reviews of care packages
- Undertaking comprehensive social care assessments and reports with detailed Support Plans as appropriate
- Working with families, carers and communities to help them make informed decisions, enabling them to clarify and express their needs and contribute to service planning
- Assessing/reassessing complex health and social care needs
- Requesting funding
- Completing Decision Support Tool (DST) to establish eligibility for healthcare
- Completing Continuing Healthcare Checklists
- Attending MDT meetings to discuss the role within the CHC team
